Duty to Report
Responsible Employees
A responsible employee is any employee who is not a designated confidential resource. This also includes Student Employees and Resident Advisors (RAs).
Any responsible employee who receives information about an incident(s) that may constitute a violation of the Nondiscrimination Policy is required to share all known information about the incident(s) with the Office for Title IX and Equal Opportunity. Responsible employees are required to report all known information about the incident(s) – including the names of the Parties involved – even when the person has requested anonymity.
If you do not know whether information you received constitutes protected status discrimination, harassment, and/or retaliation, we still encourage you to call and consult with the Office for Title IX and Equal Opportunity, or simply file a report. A representative from the Office for Title IX and Equal Office will evaluate the reported information with you and decide next steps.

Responsible Employee; How to Report a Possible Incident
If you are a responsible employee, to report a possible incident, please email a summary of the reported incident to the Office for Title IX and Equal Opportunity at:

- By submitting a completed Incident Reporting Form
Please include all known information, including:
- Your name and contact information
- The name and contact information of the student/employee who disclosed to you – and ideally, the person’s SID
- The name of the respondent, if known
- A summary of the incident
- If you do not have information for a blank, fill in N/A
If appropriate, the Office for Title IX and Equal Opportunity will send an outreach email to the person who experienced the incident. The outreach email will outline their rights and options, as well as the option of scheduling a meeting with the Office for Title IX and Equal Opportunity’s Intake, Outreach and Support Coordinator.
Confidential Resources
Confidential resources are limited to physicians, psychotherapists, professional counselors, licensed clinical social workers, sexual assault victim’s advocates, and clergy who receive information in the context of their role as a confidential resource. The following 菠菜网lol正规平台 employees are confidential resources; meaning, the following employees are not required to report information to the Office for Title IX and Equal Opportunity:
- The Campus Survivor Advocate;
- Counselors at CAPS;
- Physicians in the Student Health Center; and
- CSU union representatives who receive information from a union member in a confidential setting.
